What we do

We are a concierge practice. Every engagement starts the same way, with an assessment, so we know what we are actually dealing with. After that, our support depends entirely on what your situation needs.

When people call us

You need help now

A fall. A new diagnosis. A slow decline nobody in the family wants to name out loud. You and your family need support today.

You or a family member are planning ahead

Nothing is wrong yet. You, or a loved one, are in good health and would like to stay that way. You would rather make these decisions calmly now than under stress later.

The front door

Whole-person assessment and care plan

The assessment: We start with a clinician-led evaluation in your home that covers the whole picture.: health conditions and how they are being managed, medications, mobility, safety and fall risk, memory and judgment, the home itself and whether it still works for the person living in it, and the existing support network. We look at everything that matters at once, not one problem at a time.

The care plan: Next, weWe put together a personalized written plan with concrete recommendations. with our read of the current situation and concrete recommendations you can act on, share with family members, and hand to a doctor.

The result: One document that says where things stand, what matters most, and what to do next. No more guessing, and no more piecing it together from six different conversations.

What comes after

The care plan tells us what is needed. Some people need one of the services below, and some need all of them.

Ongoing check-ins

We come check in on a regular schedule, in person and by phone, so changes get caught while they are still small. Most emergencies announce themselves weeks in advance to someone who knows what to look for.

For example

An emptier refrigerator

On a monthly visit, the refrigerator is emptier than usual and the walker has moved to a closet. Two small changes worth a conversation now, before they become a fall or a hospital stay.

Medical coordination

We go to appointments. We make sure the doctor hears the full history and that you understand what was decided. We get specialists who have never spoken to each other onto the same page, keep the medication list current, and make sure the follow-up actually gets scheduled.

For example

Two specialists, one prescription

Two specialists each adjust the same blood pressure medication without knowing about the other. We catch it, call both offices, and come away with one plan instead of two.

Transition assistance

Moving between the hospital, a rehab facility, and home is where things often fall apart. We help you choose the right facility instead of taking the first bed offered, make sure home is safe before anyone goes back to it, and build the plan for what happens next.

For example

Meeting you at the door

The first twelve hours in a skilled nursing facility are usually chaos. We can be there on arrival, get the room set up, and make sure everything is in place before the first night.

Family decisions

Is it time for assisted living? What do we do when your brother thinks everything is fine and you do not? We have been in these conversations many times, and we bring realistic information rather than an opinion nobody asked for.

For example

To take away the keys or not

Your dad has gotten a few dings on his car recently and you're worried a larger accident could be on the horizon. Your dad is adamant that there is no issue and your brother agrees. We help moderate a conversation, bring the facts, and play a neutral third party to ultimately arrive at a decision that everyone feels comfortable with.

Crisis intervention

We show up when something major happens. We get there in person, find out what is actually going on, and start working the problem.

For example

A 2am fall

Your mom calls you at 2am after a fall, and you are three hours away. Or you are the one who fell, and you do not have someone to call to help. Either way, you call us, we are there in 30 minutes, and we stay as long as is needed.

Local resources

Elder law attorneys, home modification contractors, adult day programs, geriatric psychiatry, in-home care, and the facilities worth touring. We know who is good, because we have watched them work.

For example

One call, not a list of five names

When you need an elder law attorney, we do not hand over a list of five names. We make one call to the right person for your unique situation and brief them before you walk in.

Preventative planning

For people who are living well and want to keep it that way. We assess where things stand, identify what is likely to come next, and put a plan in place while you are the one making the decisions.

For example

Before the surgery, not after

Before a planned knee replacement, we document medications, providers, preferences, and emergency contacts, and set up the support the recovery will need, before the surgery rather than after.

If it is not on this list

These are examples of the services we offer, and this is not a menu. Families bring us all sorts of unique problems, and as senior care problem solvers, we work together to find the best solution. If something is outside our wheelhouse and we do not think we can help, we will say so and point you in the right direction.

You do not have to figure this out by yourself.
